Request an autism assessment
The first step in the referral process is completing a form that gathers key information about the person seeking an autism assessment. This includes biographical details and questions to help our clinical team determine the most appropriate course of action.
The form typically takes 30–40 minutes to complete. You are unable to save your progress so please make sure you have plenty of time and the information detailed below to hand before starting.

After submitting the form, you will be provided with a Unique Reference Number which you must state in all follow up correspondence with us. Once we receive your form we will email you to request:
- Copies of any documents you stated in the form that you could provide.
- Completion of a screening tool called the Social Responsiveness Scale (SRS) which takes around 15-20 minutes to complete - please note that you will receive this from a different email address: no-reply@wpspublish.com
- We may ask you to provide additional information to help tailor the assessment to your needs.
Please note: Completing the application for an autism assessment does not obligate you in any way to have an assessment.

Under the Health & Social Care Act 2008 and the Records Management Code of Practice (2021) we are required to keep an accurate, complete and contemporaneous record in respect of each service user and to maintain this securely. Who can complete the form?
The referral form can be completed by:
- The adult seeking an assessment
- The parent(s) of a child or young person
- Carer(s)
- An adult helping another adult seeking an assessment
- A professional making the referral
